Through La Plaza we have space, resources, and partners to create many pathways to wellness. La Plaza brings together Sonoma County leaders, partner organizations and community members. Together we (make it easier to) build community and thrive as we create and access healing activities. We support each other as we discover our individual and collective strength and ability to lead in our own lives and our community.

La Plaza’s comprehensive case management has a deep focus on community and cultural center approaches and is a holistic model that addresses individuals' needs within the context of their community and cultural background. This method recognizes that individuals are embedded within larger social networks and cultural contexts that significantly influence their well-being and ability to access resources.

At its core, comprehensive case management involves the coordination and integration of services to meet the multifaceted needs of individuals and families.
